Hospice Liaison at Proactive Healthcare Recruiters

We are seeking an experienced, well-connected Hospice Liaison is the north Los Angeles region. As the Hospice Liaison, you will partner with our Head of Hospice and our clinical teams to coordinate hospice admissions based on the criteria set out This role is critical and will be depended on to establish and maintain positive relationships with customers and referral sources within the community.

The Hospice Liaison is additionally responsible for ensuring all of the patient's needs are being met—from physical therapy to emotional support—and works closely with medical professionals and family members to ensure everyone is on the same page about what steps need to be taken next. You will work closely with patients and their families to coordinate care between the hospital, nursing home, or other facilities where they are receiving treatment and a local hospice.

Relevant Experience:

• 3+ years of experience as hospice or clinical liaison, must understand hospice/palliative jargon
• 1 year of similar sales and marketing experience in the hospice industry is preferred
• Background in nursing, particularly in palliative or hospice care.
• Strong working knowledge of all medical regulations related to hospice care.
• Bachelor’s degree in a health-related field, such as nursing, social work or psychology
• A degree in a non-health-related field, such as business or communications, and have experience in hospice care
